  4. Towards the bottom of the drop-down menu, there will be a section labeled “Police Resources.” Click on “Self Reporting Crash Form.”
  5. If the crash was reported by the police, you’ll need to fill out the “SR-1 New Jersey Electronic Online Crash form (Simpligov).” However, if you wish to modify any information on a car accident report that has already been reported, click on “SR-1S New Jersey Electronic Online Change form (Simpligov)” to add supplemental details.
  6. If the accident was not reported to the police already, you’ll need to click on the “New Jersey Self-Reporting Crash form (SR-1).”
